In phonosurgery, every invasive intervention into the integrity of the vocal fold, especially the lamina propria, is potentially harmful. Damage of this structure mostly leads to scar formation, which per se is not negative, but is associated with stiffness. And stiffness almost always means impeded vibrational ability, leading to audible hoarseness. However, in the damaged voice, it nevertheless might be worthwhile to perform a surgical intervention to avoid other and more harmful forms of progressing stiffness by damaging disease or disorder. In this presentation, the author will demonstrate the basic guidelines for surgical interventions and focusses on vocal fold polyps. He shows how to avoid additional trauma to the delicate structures of the vocal folds. Tips and tricks for the advanced surgeon will be given and demonstrated with video cases.
